"Builder says that one of the building is approved till 4th floor only and approval process for remaining floors is under progress."

My questions :
1. Is it usual practice to get approval in stages?
2. does it give some hint of wrong plan and builder is planning to revise it?
3. what may be the possible reasons for not getting it approved?

Pls note :
1. the the slabs and brick work is completed till 12th floor already.
